President’s Message – Getting Involved

Anyone who stays abreast of current events knows that many obstacles threaten the future of the practice of medicine as we know it. To succeed as physicians, it is no longer enough just to graduate from medical school with honors, and complete the residency/fellowship program of our choice in a great academic institution. That is the easy part. Once in practice, there are a number of things to worry about, many out of our control. There are malpractice issues, and reimbursement issues. There are licensing issues. There are legislative issues that may affect us at a local and national level.

There are a great number of people and agencies in this country looking at the status of health care, trying to fix what they see as not working right. Topics such as Medicare, patient’s rights, prescription-drug coverage for the elderly, indigent care (to name a few) are once again on top of the agendas. The Board of Medical Examiners is considering instituting an exam for re-licensing purposes.

You may be surprised to know that physicians as a whole have very little to do with the many decisions made surrounding health care in this country. Is there anybody on our side?

In order to ensure our viability, we have to unite and get involved. That is one of the purposes of our local and state medical societies. Regretfully, however, while a significant number of physicians in Clark County belong to the medical society, only a small percentage actively participates in its affairs. Programs organized by our County and State associations are poorly attended by the physician members. Unless we start making an effort to attend meetings and to voice our opinion, we could wake up one morning to changes we may not like.

This year the Nevada Legislature is considering several Health and Medical related bills that will impact our practices. The Nevada State Medical Association needs our support at all times to convey our messages and concerns to our elected officials.

The NSMA will hold its 97th annual meeting May 17-20 in Litchfield Park, Arizona. The most important part of this gathering will be the meeting of the House of Delegates. The Clark County Medical Society’s delegation (up to 30 physicians) will be there to represent you. As of this writing, however, we do not have a full delegation. Last year, 13 delegates from CCMS attended, while we were entitled to 20 delegates. Although it may be too late to volunteer as a delegate this year, it is not too late to plan on attending the meeting. You will find it stimulating; and who knows, you may want to get involved next year!

·        Sunrise Medical/Surgical Specialists obtained a donation of $500 from Bristol Meyer for the 2001 Charity Gala. Proceeds from the January event benefited the CCMS Medical & Nursing Scholarship fund.

·        UMC has teamed up with the State of Nevada to aid families in sigining up their children for the Nevada Check Up program. Nevada Check Up provides low-cost, comprehensive health care coverage to uninsured children who are not covered by private insurance or Medicaid. The program covers both inpatient and outpatient hospital visits; x-rays; dental, vision and hearing; prescription medications; immunizations; physician laboratory and ambulance services; chiropractor visits; and more. ·        Southwest Ambulance held a grand opening ceremony in January for their new facility. Their station is located at 4640 South Arville, Suite G. For more information, call 650-9900.

·        CCMS welcomes new staff member Deborah Spencer as Public Relations Coordinator. Spencer most recently served as Public & Community Relations Coordinator for Sierra Health Services and has a degree in Journalism from Southwest Texas State University.

New Needlestick Regulations Take Effect April 18

More than 8 million health care workers in the United States work in hospitals and other health care settings, and every 32 seconds, someone falls victim to an accidental needlestick injury.  Such injuries can lead to serious or fatal infections with bloodborne pathogens such as hepatitis B virus, hepatitis C virus, or human immunodeficiency virus (HIV).

According to a report published in March 2000 by the Centers for Disease Control and Prevention, depending on the type of device used and the procedure involved, 62 to 88 percent of these sharps injuries could potentially be prevented by the use of safer medical devices. 

Armed with these statistics, and addressing the emotional and financial impact these needlesticks represent, a group of legislators developed and presented what has since become the Needlestick Safety and Prevention Act.  The Act was passed unanimously by Congress then signed into law by President Clinton on November 6, 2000.  It was finally published in the Federal Register on January 18, making it applicable to all healthcare providers across the nation. 

As a result, your practice must implement the following changes on or before April 18, 2001:

·        Create or update your Bloodborne Pathogens (BBP) Exposure Control Plan to reflect all provisions of the revised Standard

·        Complete written assessments on all “safer medical devices” you evaluate for use within your practice.  Frontline healthcare workers must be included in the evaluation and selection process.

·        Utilize “safer medical devices” where such devices are found to be effective in eliminating or minimizing occupational exposure. 

·        Develop a sharps injury log and tracking system for the recording of percutaneous injuries from contaminated sharps.

·        Conduct employee training in the use of “safer medical devices” and safer work practices, particularly as new devices are introduced into the work setting. 

·        Implement a quality assurance program to verify that your systems and protocols are positively impacting your work setting.

As a result of recent publicity on this topic, and the potential impact of noncompliance, focused enforcement of the revised Bloodborne Pathogens Standard has become a directed priority for OSHA Enforcement Officers.  There is no list of exceptions or exemptions.  Employers must review and consider commercially available devices to determine whether they are effective in reducing occupational exposures to the lowest feasible extent.
